Treatment of the malpositioned implant (too buccal and too deep) in the esthetic zone: Atraumatic explantation + CTG * 3D Bone Regeneration * Delayed implant placement + CTG
23 year-old woman who had received an immediate implant 3 years before and experienced periimplantitis. Due to the malposition of the implant and the papilla loss in the adjacent central incisor, the regenerative treatment was contraindicated because the malposition of the implant was the genesis of the problem and even if the regeneration had obtained the final outcome would be a esthetic failure and the prognosis would always be poor.
The protocol made was: atraumatic explantation using an implant retriever and socket preservation with a connective tissue graft. 3 months later a 3D Bone Regeneration procedure was made using the "casket" technique described by Prof. Khoury. 6 months later a 3,5 mm implant was placed and simultaneously a connective tissue graft was added. 3 months later the implant was loaded with a zirconia-porcelain screw-retained crown. 1 year later clinical and radiografphic status of the implant was healthy.
